South East Essex Community Diabetes Service

Summary

The Community Diabetes Service is a nurse led service that facilitates self-management, enabling people with diabetes to make the necessary adjustments to remain well, reducing mortality, morbidity and the need for hospitalisation.

Service Information

The service is delivered through a combination of face to face clinics located at various sites in the community, telephone or ‘virtual’ clinics, domiciliary visits and various education programmes for people living with diabetes to best meet the needs of the individual.

The Service also provides care to patients in nursing and residential care homes.

Self referrals only accepted for Type 2 Structured Education Programme.

MSE SWEET – South West Essex Education Training for Type 2 Diabetes

The MSE SWEET – South West Essex Education Training is for people with type 2 diabetes and is an essential part of diabetes management.  

Taking part in the training can help people with diabetes improve their knowledge, skills and confidence. Enabling them to take control,  integrate effective self-management into their daily life and overall improve their quality of life.

People who go on these courses often feel more confident about managing their diabetes, making healthier food choices, looking after themselves and meeting other people with diabetes. If you’re supporting someone with diabetes, you’re welcome to join them on a face-to-face course.
